You may re-open CHIP when:
The case was closed because of an administrative error. Re-open the case back to the date of closure. Do not require the enrollee to complete a new application.
The case was closed for failure to complete the renewal. The enrollee must return the forms, verifications and be interviewed, (if required), by the end of the renewal month or by the end of the month immediately following the review month. (See section 704) Do not require the enrollee to complete a new application.
The case was closed for failure to provide requested information or verification and the information is received by the end of the month of closure or by the end of the month immediately following the closure month. Do not require the enrollee to complete a new application.
The case was closed because a parent was activated to military duty so the children became eligible for military insurance coverage. If there are still months left on the original certification period, do not require a new application. If the certification period has expired, the individual must complete an application and provide verification of their current circumstances.
An enrollee became eligible for Medicaid because they were removed from the home and placed in foster care. When the child is returned to the home, if there are still months left on the original certification period, do not require a new application. If the certification period has expired, the individual must complete an application and provide verification of their current circumstances.
An enrollee was removed from CHIP and added to Medicaid for the purpose of qualifying a parent for a Family Medicaid program that requires a dependent child to be included.
The case was closed for failure to pay their quarterly premium and the family pays the premium by the end of the month immediately following the closure month.
The case was closed for failure to pay quarterly premiums, and a child is being considered for CHIP enrollment within one year of the closure, and the family pays the back premiums. All children in the family who qualify may be enrolled, including those who were enrolled when it closed. The effective date of enrollment for these children is the same as the effective date for the new child.
