A review is a re-evaluation of the CHIP eligibility factors that are subject to change to decide if eligibility still exists. The CHIP eligibility agency is responsible to complete a review every 12 months.
If a change is reported after the review form is mailed out, but before the review has been processed, consider that information when completing the review.
All enrollees have the right to register to vote at review. See Section 101.
There are two types of review forms and processes, a mandatory review and a simplified review.
Mandatory Review: A mandatory review form will automatically mail if the eligibility system recognizes certain criteria or changes during the 12 month certification period. A mandatory review will generate for any of the following reasons:
Previous two reviews were simplified reviews (a mandatory review must occur every 36 months).
Household membership changed.
CHIP eligible child turns 19.
Income type is Self-employment.
New employment; a job change occurred.
Income changes; the income amount changed from the amount used to certify CHIP.
Adjusted Gross Income (AGI) was used as verification of income to certify CHIP. (See Section 704-9)
Countable income exceeds 180% of Federal Poverty Level.
Countable income is less than 100% Federal Poverty Level.
An enrollee who receives a mandatory review form must either complete and return the review form to the eligibility agency or contact the eligibility agency to complete the review by telephone. If the enrollee fails to respond to the review by the last day of the month following the review month, they must reapply.

Simplified Review: When no changes have occurred or have been reported during the 12 month certification period, the enrollee will be sent a simplified review form.
The client is not required to send in the form or take any further action unless any of the preprinted information on the form has changed.
If changes to the preprinted information have occurred, the enrollee must follow the same process as described above for a mandatory review.
