704  Eligibility Review

Effective: April 1, 2011 - January 31, 2012

Previous Policy

A review is a re-evaluation of the CHIP eligibility factors to decide if eligibility still exists.  The CHIP eligibility agency is responsible to  complete a review every 12 months.  If a change is reported after the review form is mailed out, but before the review has been processed, consider that information when completing the review.

All enrollees have the right to register to vote at review.  Ask if they wish to complete the voter registration form, or check to see if they have marked that they want to register on the review form.  See Section 101 for additional information on voter registration.

There are two types of review forms and processes, a mandatory review and a simplified review.  

Mandatory Review:  A mandatory review form will automatically mail if the eligibility system recognizes certain criteria or changes during the 12 month certification period.  A mandatory review will generate for any of the following reasons:  

     

An enrollee who receives a mandatory review form must either complete and return the review form to the eligibility agency or contact the eligibility agency to complete the review by telephone.  If the enrollee fails to respond to the review by the last day of the review month, they must reapply.

Simplified Review:  When no changes have occurred or have been reported during the 12 month certification period, the enrollee will be sent a simplified review form.  When an enrollee receives a simplified review form, they are not required to send in the form or take any further action unless any of the preprinted information on the form has changed.  If changes to the preprinted information have occurred, the enrollee must follow the same process as described above for a mandatory review.
