Obsolete 0118 - 705-1 Who Must Provide Verification

Previous Policy

The eligibility agency must use available resources to determine CHIP eligibility.  It is the responsibility of the applicant or enrollee to obtain acceptable verification of eligibility factors when the eligibility agency cannot access the information.  Help the enrollee to get the verification if they need help.

If the verification is not available, the case worker will make necessary contact on behalf of the enrollee to verify this information.  Document these contacts in the case record.  If a third party is unwilling or unable to cooperate with an agency request, the worker will investigate other methods of verification with the enrollee.
