705-1 Who Must Provide Verification?

It is the responsibility of the applicant or enrollee to obtain acceptable verification of eligibility factors.  Help the enrollee to get the verification if they need help.

If the verification is not available, the case worker will make necessary contact on behalf of the enrollee to verify this information.  These contacts must be documented in the case record.  If a third party is unwilling or unable to cooperate with an agency request, the worker will investigate other methods of verification with the enrollee.  Documentation of the verification method must be noted in the case record.
