Applicants or enrollees who are required to provide a Social Security number but do not have one must apply for a Social Security number. They must provide verification that they have applied before they can receive or continue to receive CHIP. Any of the following are acceptable verification that an application for a Social Security number has been made:
A form 5028, 2880 or a computer letter from the Social Security Administration.
A birth certificate or a form SSA-2853 showing that the parent asked to apply for a Social Security number when they provided the birth registration information.
The birth certificate must include the child's name, an indication the SSN was requested, and the signature of a hospital official.
The form SSA-2853 must include the child's name and a signature of a hospital official.
Do not deny, delay or discontinue CHIP while waiting for a SSN to be issued.
Individuals who fail to provide their SSN after one has been issued are not eligible. Provide 10-day advance notice before closing a case or removing a person from the coverage.
