Policy
The Office of Managed Health Care's Health Program Representatives (HPRs) will provide CHIP education, help the members understand their CHIP benefits, and assist them with choosing a health and dental plan.
HPRs provider information on CHIP-covered services, including primary care, specialists, dental services and mental health services.
Any questions about CHIP services or covered benefits should be referred to the HPR phone number 1-866-608-9422 or members can contact their health or dental plan.
Additional information about benefits, providers, forms, and cost sharing (co-pays, deductibles, etc.) can be found online at: www.health.utah.gov/chip/.
CHIP is administered by the Utah Department of Health and Human Services and contracts with medical and dental plans to provide health and dental services.
Individuals who are determined eligible for CHIP must enroll in a medical and dental plan before they can access their benefits or services. All CHIP members will be auto enrolled in their dental plan.
Members must enroll in a medical plan within 2 weeks of approval or a plan will be assigned to them.

CHIP dental benefits are provided through Premier Access and members will be auto enrolled in the CHIP dental plan.

Note: When selecting a plan via email, they must include their plan selection, case number, name of the primary individual on the case, and their contact information.

The plan selection form is included in the CHIP member guide and can be accessed online at: www.health.utah.gov/chip/resourcematerials.htm.
Once the member is enrolled in a health and dental plan, they will receive a plan specific member handbook from their selected plans. The member handbook includes an explanation of covered benefits and a provider directory.
Under certain circumstances a member may change their health plan outside of the annual open enrollment period. Enrollees must contact the HPR with any specific plan requests by phone at 1-866-608-9422.
