With few exceptions, applicants and individuals who are enrolled in CHIP must provide their correct Social Security Number (SSN) or provide verification that they have applied for an SSN. This is true when they first apply or ask to add a person to their case. A Social Security card or other verification of the Social Security number is not required unless the number cannot be verified by computer match. If an applicant does not have a Social Security number, he or she must apply for one and provide the number once one has been assigned.
